Community Health Needs Assessment (CHNA)

Tarzana Treatment Centers, Inc. (TTC), a 501(c)(3) organization, has posted Community Health Needs Assessment (CHNA) documents as required by the United States Affordable Care Act (ACA). TTC operates an acute psychiatric hospital in SPA 2, and is therefore required to participate. The documents are available for viewing, printing, and downloading at the links below on our website:
There are two documents linked on this page; The CHNA and the Implementation Plan.
TTC staff members, 32 in all, collaborated to create this document. It is a comprehensive survey of the San Fernando Valley and Santa Clarita Valley community, and includes health statistics and demographics. Tables and findings are provided for review, as well as information about how the survey was conducted.
Data from the CHNA survey is used in this document to highlight the health needs of the community and how TTC provides services for them. Details include current services and those we plan to implement, where we offer referrals, and services that we don’t intend to provide.
Most important, the Implementation Plan explains how TTC services will help those in need. It addresses the overall concern of community health needs, and concludes with how TTC will work to improve them.
