Chronic Care Management
Chronic Care Management is a program that Tarzana Treatment Centers (TTC) offers to all Medicare patients. Our goal is to make sure you get the best care possible from everyone involved with your care. We can help coordinate your visits with other doctors, facilities, labs, radiology, or other testing; talk to you on the phone about your symptoms; help you with management of your medications; and provide you with a comprehensive care plan.
TTC uses a team-based approach to care, meaning you have more than one person who takes care of your medical needs. Each Care Team is led by a licensed medical provider. Other staff from your Care Team will talk to you or handle issues related to your care, but your assigned licensed medical provider will supervise all care provided to you by TTC staff.
Under the Chronic Care Management program, you will receive a comprehensive Care Plan from our practice to help you understand how to care for your conditions so that you can be as healthy as possible. And the care team is available for you to contact 24/7 to discuss your urgent care needs.
