LA Care eConsult Project

A project is now underway in Los Angeles that will improve the speed and efficiency with which Los Angeles residents receive medical care.  LA Care started the eConsult project in preparation for the expected increase of new patients at local clinics due to health care reform and other program implementations.  Results from a successful preliminary pilot project suggest that eConsult will result in reduced wait times and fewer unnecessary face-to-face visits with specialists.

Tarzana Treatment Centers is pleased to be participating in the eConsult project.  It is another way we remain on the cutting edge of healthcare information technology.

Technology in healthcare is advancing with big leaps since the turn of the century.  With greater security, telemedicine is fast becoming a necessary tool for health care providers.  For example, store-and-forward data transfers allow for simple and secure communication between providers.  LA Care’s eConsult project uses store-and-forward technology via a web interface for Primary Care Providers (PCP) to exchange patient information with specialists.

Instead of giving a patient a direct referral to a specialist, the PCP sends an eConsult request to the specialist.  The specialist is notified of the request and can  ask further questions of the PCP and send treatment information back to the PCP or recommend that the patient come in for a visit with the specialist.  This is  an over-simple example because some specialties will require more back and forth communication between the PCP and specialist..    In many cases, though, patients will no longer need to wait for an appointment with a specialist to get input from a specialist about their condition..

An  18 month preliminary pilot project conducted by LA Care resulted in an approximate 60% reduction in appointment wait times.  About half of the specialty referral requests were resolved using eConsult rather than requiring a face-to-face visit with the specialist.  Specialists were able to respond quickly, and PCPs felt better equipped to address their patients’ specialty needs.  More details about the project can be found at this link:

New HIV Medical Care Clinics in the Antelope Valley

Tarzana Treatment Centers, Inc. in Los Angeles (TTC) plans to open one HIV medical clinic in Palmdale and a second clinic site in Lancaster in the Antelope Valley (AV).  Tarzana Treatment Centers will provide low and no cost primary medical care, specialty care, dental care, case management, mental health, and transportation assistance to Antelope Valley residents living with HIV/AIDS.  This is yet another example of our commitment to provide integrated health services to underserved individuals in Southern California.

Tarzana Treatment Centers already treats thousands every year for medical, mental health, substance use, and behavioral disorders.  Our expertise doesn’t stop with providing a wide array of services, though.  We integrate services so patients can receive all of them “under one roof”.  Even if patients need long-term residential alcohol and drug treatment, for example, we provide the health care services they need so treatment is not interrupted.

The original goal of health care integration was to provide worry-free treatment to substance abuse and behavioral health patients, and it has evolved to include most common health care needs.  By applying this to HIV/AIDS services we set the stage for patients to live long and fruitful lives.  This is because many of them live with substance use and/or mental health disorders which can deter them from getting the treatment they need.  By integrating health services, we can treat all issues for each patient.  And in most cases, we can treat them all at the same time.

With an expert staff of medical and behavioral health providers, our new clinic sites in the Antelope Valley will serve 350 patients living with HIV/AIDS in the first year of operation.  As with the HIV services we provide in the San Fernando Valley, patients will have access to needed medications, medical care, mental health treatment, alcohol and drug treatment, and more.  The Office of AIDS Programs and Policy (OAPP) grant that funds the clinics allows us to also provide services for oral health needs, lab work, and transportation as well.

The inclusion of case management for medical and mental health is what most benefits our patients living with HIV/AIDS.  Individuals who have medical and mental health issues that require treatment, for example, can schedule appointments in the same time frame at one location.  A Case Manager oversees patients’ treatment to ensure they get all of the services they need.  This means convenience and better health for HIV positive individuals seeking health care at Tarzana Treatment Centers.

Comprehensive assessment services at Tarzana Treatment Centers give our Case Managers a clear picture of the health care needs for each new patient.  After medical and psychological assessments, lab tests, and basic interviews, we know the primary and secondary issues that patients face.  We can then admit patients into our various programs as needed, and Case Managers can follow their progress and adjust treatment as may be necessary.

Telemedicine Uses – Teledermatology

Teledermatology is a health technology tool used by dermatologists all over the world. With this, dermatologists are able to reach out to patients in need of care from thousands of miles away- distance is not a barrier to treatment. Using teledermatology makes it possible for a patient to receive treatment from a dermatologist within only a few hours or days, instead of having to wait several weeks or months. Another benefit of using teledermatology is efficiency – it allows dermatologists to treat more patients in less time.

One teledermatology method is to participate in a live online interactive treatment session with the patient (web camera, computer with internet connection). Another way is through email. The dermatologist views a high-resolution image sent to their email, usually by the patient’s primary care physician. In many cases, the doctor can view the patient’s ailment and give a diagnosis for treatment,then prescribe medication to the patient. All of this can happen without the necessity of a face-to-face visit. Additionally, the dermatologist can give a consultation through email or phone.

Teledermatology is very beneficial because a patient can begin treatment much sooner and be on their way to better health. Without teledermatology, unfortunately for many patients, the ability to see a dermatologist face-to-face usually involves having an appointment date in the far future, a long wait time in a doctor’s office, or not at all due to travel issues from home to the doctor’s office. Teledermatology is a convenient and efficient way for a patient to receive much needed healthcare.

Telehealth Technology Provides Security and Simplicity

Tarzana Treatment Centers in Los Angeles provides telemedicine services to qualified patients in alcohol and drug treatment, mental health treatment, and primary medical care.  After careful research of the technology available, we chose to partner with MDLiveCare in Florida so we could deliver services conveniently and securely at affordable rates.  The technology was a key in making our decision considering what our patients would need in order to use these services.

During our research of available telehealth technologies for video conference sessions, we were happy to discover that most of our services could be delivered through this medium.  Assessments of new patients entering addiction treatment or mental health treatment can be completed before they come to one of our locations.  This allows our staff to better prepare for their arrival, and saves patients critical time in on-site admissions time.  It can also be used for outpatient services for some patients.  We can also see qualified patients in primary medical care from their homes and offices.  

The MDLiveCare technology is browser-based.  This means that patients simply need to open Internet Explorer, Firefox, Chrome, or whatever they use on a daily basis, and log in to their account.  Once logged in, a simple interface makes it easy for them to schedule appointments with Providers, send and read secure messages with them, and view records.  Anyone who browses the internet should be able to use the interfaces we provide, known as MDLiveVisit and TherapyLiveVisit.

Much of the technology used for telehealth video conferencing today would be considered complicated by the everyday computer user.  Some require special software that, putting it simply, directly connects one specific computer with another over long distances.  This adds work for the patient in setting up the software, which may not go well and lead to a need for technical support.  Also, if the program crashes there is no backup means to connect to a session, creating a delay in services.  These are issues that patients do not want to deal with when they need health care.

The MDLiveVisit and TherapyLiveVisit interfaces that are used by Tarzana Treatment Centers require no initial setup on patient computers.  They only need a login ID and temporary password to get started.  Literally within minutes, patients can set an appointment to see one of our health care providers.  And, if the browser they are using happens to crash, they can switch to another to start or resume a session.  All of this simplicity is delivered with the same or better security as those described above.

The LiveVisit application was started using the popular Flash Player to deliver video.  This meant that almost anyone already had the software needed to send and receive video before they used LiveVisit.  To improve services, a switch was made to using the VSee video format in LiveVisit.  The new format uses less bandwidth, meaning that even patients with very slow internet access can send and receive smooth video.  Patients still do not need to install bulky software to use this advanced video format.

We chose to partner with MDLiveCare and use the LiveVisit telehealth applications because the simplicity and security satisfied our high standards in delivering services to patients.  The goal was to make getting treatment easier for patients, with almost no chance that problems will occur.  No special equipment is needed, either, other than an inexpensive webcam since high resolution is not necessary in LiveVisit.

Senior Alcohol Abuse - Damaging Effects

Senior alcohol abuse is often more difficult to correct than with any other group in America.  By age 60, most individuals are set in their ways and don’t feel the need to change anything.  In fact, many seniors are aware of the risks they face with heavy drinking and continue seemingly without care.  Loved ones may feel helpless, but solutions exist.

About 40% of those over 60 say they drink alcohol, with almost one-third of them admitting to binge drinking and heavy drinking, or alcohol dependence.  These statistics come from a 2007 report at the Substance Abuse and Mental Health Services Administration, Office of Applied Studies website.  This means that about 1 in 10 seniors at least binge drink on occasion, which can be very dangerous to their health and that of others.  

Many external changes happen with individuals when they reach age 60.  Retirement is imminent if it has not already occurred, friends begin to move away or pass away, and family members may even suggest a change of residence.  Worst of all, the mind and body begin to show signs of aging, meaning that doctor visits become common and more medications need to be taken.  All of these can be terribly stressful on anyone when they are grouped into a few short years.

Boredom, loneliness, and a sense of powerlessness can each lead anyone to drink alcohol.  Whether it is a time-filler or an escape from negative feelings doesn’t matter.  The end result is that it works and leads to earlier drinking times and less time outside the home.  Senior alcohol abuse may even be a purposeful celebration of the golden years.  The daily party begins with joy, but can quickly turn to injury or a fatality.

This can all be very harmful for seniors due to interactions with medications, a higher risk of falling, and aging major organs.  If alcohol dependence sets in, the effects on the body and brain are more detrimental to seniors than anyone else.  The celebrators, in freedom, usually drive under the influence.  Yet most seniors who abuse alcohol either hide it or justify it, and accept the risks.

Family members who see the alcohol abuse will often accept the behavior as a rite of passage.  Considering the years they have lived, why would a loved one deny what seems well deserved? And if nobody gets hurt, all is okay.  That is, until somebody gets hurt or health problems develop.  Some seniors will stop the behavior at this point, but most will continue to drink heavily and probably more.

There are 2 things that loved ones can do at this point.  The first is to strongly suggest alcohol detox and alcohol treatment.  But most people in their 60’s today will see that as a sign of weakness and refuse, and family members generally won’t force the issue.  Doing so may get the elderly alcoholic into treatment, but he or she will only benefit if self-motivated.  So keep suggesting, with love, and let them make the decision.

Second, you can set boundaries and stick to them.  If they refuse to quit drinking or cut down, suggest accompanying them to the doctor so you can discuss medication interactions.  Let them know you understand that they are not concerned with their own health, but you are concerned with the health of others including yourself and younger members of the family.  It may be hard, but you must be firm, with love, and continue to suggest alcohol treatment.

To the younger generations, seniors who refuse to change these behaviors seem stubborn.  However, like any other individual who engages in alcohol abuse, the bottom line is that they are escaping from emotions.  Instead of arguing and treating them like children, it is best to discuss feelings as much as possible.  Avoid forcing the issue of senior alcohol treatment, and use gentle, loving nudges.
